Instructions for Weekly Player
Submission Form
Division 1- Thursday Matches
                        Form Due Tuesdays by 7:00 PM

Division 2- Wednesday Matches
                         Form Due Mondays at 7:00 PM
 
  • Format Names as Follows: Firstname Lastname
    e.g., Dave Paruti  (Dave [SPACE] Paruti)
  • When Typing Names- Scroll Over the Label in  the Box and Then Type in Player's Name
  • Make sure you populate Captain's Name, Team, eMail Address & Match Date
  • Check off Division 1 or 2









     
  • Enter Players Names for each Match Group using the Format Shown above
Alternates:
You are able  to name 2, 3 or 4 Alternate players.
  • For 2 Alternates (Required), Use the Alternates 1 & 2 Fields...
  • If you want 3 or 4 alternates, Use the Alternates 3 & 4 Fields
  • Format:  firstname lastname in first box & firstname lastname in second box.
